Character Application

Here at Tooth and Claw, we require our members to fill out an application form for their characters. It's quite simple, and anyone can try to improve the design of the application as long as they don't edit the information(ex. Age:, Gender:, Breed:). Below is the character application, and it's very impossible to really make any errors.
